IMPORTANT INFORMATION REGARDING EYE EXAMINATION REQUIREMENTS:

For all students entering Kindergarten or any student who is enrolling in a public, private or parochial school for the first time are required by the lllinois Department of Public Health to have an eye examination. Each of these children are required to provide proof of having an eye examination by a physician who performs eye examinations or an optometrist within the last year, before October 15, 2008. The required form can be used through 10/15/08. It is available online and can be accessed through the link below.
