Health Requirements

 MEDICAL RECORDS REQUIRED FOR REGISTRATION

  March, 2012

  Peotone CUSD 207-U Parents and Guardians:

 
   STUDENT HEALTH INFORMATION SHEET

  In order to better serve your child’s medical needs, we ask that each year; all parents complete a
  Health Screening Information sheet for any child who attends Peotone 207-U schools.   In addition,
  please notify your school nurse throughout the school year if there are any medical changes or updates
  for your child.


 
    SCHOOL PHYSICAL REQUIREMENTS

   Students entering kindergarten, sixth and ninth grades are required to have a completed Illinois
   Certificate of Child Health Examination Form (school physical) on file for the upcoming 2012-2013
   school year. This form must be completed and signed, including the date, by your physician.  Your
   physician must also mark the physicial education and interscholastic sports boxes to be complete.
   There is a health history portion to be completed by the parent, including a signature and date.  
   Forms that do not have the required information, including signatures, will not be accepted as
   completed forms. These forms are due on or before the first day of school.  If a physicial
   is not received by that date, your student will be excluded from school until a physical is on file in
   accordance with Illinois School Code 11:250 and 11:252.  Please note, this includes up to date
   immunizations.



   SCHOOL DENTAL REQUIREMENTS

   Students entering kindergarten, second, sixth, and ninth grade are required to provide a Proof
   of School Dental Examination form.  These forms are mandatory and due on or before the first day
   of school. You may use any dentist that you choose for this examination.



 
   EYE EXAMINATION REQUIREMENTS

   Students entering kindergarten or upon first entry into an Illinois school beyond kindergarten
    (i.e.grades 1-12) are required to have an eye examination.  Examinations must be performed
    by a licensed optometrist or medical doctor who performs eye examinations, as specified in
    Illinois Department of Public Health administrative rules.  He/She shall complete and sign the
    Eye Examination Report form, as designated and available in the Illinois Department of Public
    Health and the Illinois State Board of Education website.



 
    MEDICAL GUIDELINES

   Peotone School District employs two nurses for the six buildings.  Because of this reason, it is
   impossible for only nurses to administer medication to students should they require them.  The
   purpose of these guidelines is to assure that your child safely receives the medication they need
   whether it be in the presence of the nurse or other support staff.

   Please do not hesitate to contact your child's school nurse if you have questions about their
   physical form, dental form, immunization needs, or to schedule a meeting with her.  The nurse
   can be reached by contacting your child's school office.
